Engagement and Retention Specialist

The Engagement and Retention Specialist is an expert in the benefits, qualifications, and services that are offered by the care management programs operated by BHN in addition to other. Accountable Care Organizations (ACO) and Manage Care Organizations (MCO). The Engagement and Retention Specialist provides high quality identification, outreach, and engagement services to target care management populations as well as markets the care management services to partner agencies.
Your experience, knowledge and great skills will support the One Care program with the following:
- Assisting individuals with technology needs through helping individuals obtain a device necessary for connection with providers, education on using technology, as well as assists with obtaining internet services through the ACP (Affordable Connectivity Program, etc.).
- Providing telephonic and in-person health interventions both medically and behaviorally
based on sound clinical theories and principals.
- Maintaining flexibility in providing services in settings other than Clinics (e.g., client
homes, health clinics, and other community settings)
- Utilizing reports within different BHN systems to identify eligible members for BHN One
Care Health Home as well as other Care Management programs; contacts members and
their providers to assist with enrollment protocol.
- Giving presentations on the benefits of BHN Care Management programs to internal and
external referral sources included but not limited to: BHN staff, BHN integration sites,
health fairs, community agencies that provide social services to eligible populations. (Homeless shelters and other agencies.)
- Working with partner health centers and BHN medical integration staff on identifying unmet behavioral health needs in their dual eligible population and assists those individuals in getting connected to BHN for appropriate services and/or getting enrolled in for One Care and/or other eligible Care Management programs.

What are we looking for in an Engagement and Retention Specialist?
- Minimum of one year experience working with the above populations.
- High School or GED equivalent required; bachelor’s degree preferred.
- Knowledge of mental illness, the interface between mental health and medical conditions and systems, substance abuse, and care management.
- Knowledge of insurances and navigating insurance websites.
- Professional demeanor and phone skills in communicating with members and care team members.
- Demonstrates cultural competence in working with individuals with a variety of cultural backgrounds.
